Methods of Identification: Run Phenotypically Similar Panel Cells

When reactivity has no distinct pattern, the patient's serological phenotype for common antigens will aid in determining if the reactivity is due to multiple red cell antibodies or if there is an alternative antibody present. Other antibodies with similar characteristics to "HTLA" antibody/antibodies include a warm autoantibody, cold agglutinins, drug related reactivity (such as Daratumumab), etc.
  • If the patient's phenotype is obtained, and panel cells that closely match (phenotypically similar) are non-reactive, then you likely have determined that the patient has made several antibodies to common red cell antigens and should investigate accordingly.
  • If the phenotypically similar panel cells demonstrate similar reactivity to the initial panel (as seen below), then further investigation is required.
    • For this example, the patient types as O+ and C+ E- c+ e+; K-; Fy(a-b+); Jk(a+b+); M+ N+; S- s+

Note that two of the three phenotypically similar panel cells were weakly reactive, which tells us that more investigation is needed.
