In addition to data sources originating from outside the laboratory, non-conformities may be identified from within the laboratory or organization.
Some examples of internal sources of NCEs include:
- Internal audit or interim mock inspection findings
- Discovery of a defective medical (laboratory) device
- Quality indicator data
- Quality control data
- Re-work data
- Turnaround time (TAT) reports
- Training and competency experiences
- Staff observations
- Communication breakdowns within the laboratory
- Mistakes and problems within the laboratory’s work processes
