Example, continued-- Assessing Risk

After observing the process and comparing what was observed to what is expected, it becomes clearer where errors can, and do, occur. At this point, we will identify and assess the risks. The tools that we will be using are:
  • A fishbone diagram that was created by the assessment team to identify potential factors that could lead to the identified errors (identified through research and process observation)
  • Risk matrix to rank the identified risks from least likely to cause harm to most likely to cause harm
  • Process map, colored for visualization of areas needing focused attention (shown on the next page)
