The donor history questionnaire (DHQ) has been developed to help potential donors and donor centers determine if it is safe for those who want to donate blood and blood products to do so and to ensure that the products collected are safe to transfuse. The DHQ was a collaborative effort and included input from the following organizations:
- Association for the Advancement of Blood & Biotherapies (AABB)
- American Red Cross (ARC)
- America's Blood Centers
- Armed Services Blood Program
- Canadian Blood Services
- Centers for Disease Control and Prevention (CDC)
- Food and Drug Administration (FDA)
The DHQ must be completed on the same day of the scheduled donation prior to the collection of any blood product. It can be self-administered or administered by a trained professional (donor historian). If self-administered, the questionnaire must be reviewed by a donor historian. If the DHQ is being administered by a donor historian, that interview process must be performed in an area of the donor center where there is privacy.
