History of Present Illness (HPI) for Nicolás

Nicolás is an eight-month-old patient who is brought to the free clinic by their mother. They have both recently immigrated to the United States from Mexico. The mother states that Nicolás had a home birth in Mexico and has been healthy as far as they know. The mother had no prenatal care in Mexico but states that they did take prenatal vitamins during pregnancy and stopped smoking when they found out they were pregnant. The mother reports that Nicolás was born at full term and was not aware of any sexually transmitted infections.
Nicolás has been exclusively breastfed since birth and is making a normal amount of urine and stool-filled diapers every day per the mother. The patient’s mother wants Nicolás to receive their vaccines and have the appropriate blood work recommended for their age group.
