Current Address

As a laboratory licensee, you are required to maintain a primary practice location and a current mailing address with the Board of Clinical Laboratory Personnel. This is so that any notices required by law may be served to you by the Board or its agent. You must designate which of the two addresses should be used to serve you with any notices or other documents. If you move, or change your practice location (even if it is outside of Florida), you must notify the Board within 60 days of this change. You may do this either by mail or electronic methods. See the Board’s website, https://floridasclinicallabs.gov/. for procedure and change of address form. (Refer to F.A.C. 64B3-1.006)
